The folder can also be your new entry point to online services such as booking an appointment at your health center or checking your status on waiting lists.
It also offers you useful links with self-care tips, as well as access to our search engines for health centres and on-call pharmacies.
The Personal Health Record is the result of digital information generated by the centers where you receive care, and that which you consider necessary to provide, to complete your medical history.
The app includes a specific section on the COVID-19 coronavirus.
What do I need to access My Health Record?
– Be 16 years or older
– You must go to your health center and make the request in person, proving your identity.
– Have an email address and a mobile phone number to receive SMS.
– If you already have credentials from having previously made a request to access the Personal Health Record website, you do not need to do so again.
